PRIVACY POLICY

This Internet Privacy Policy (the “Policy”) is intended to inform you of the types of information Innointuition LLC DOB as Rehabwheel (“Rehabwheel” or “we” or “us”) collects, as well as our policies and practices regarding the collection, use, protection, and disclosure of that information that we receive from visitors to the web pages at www.Rehabwheel.com (the “Site”). This Policy applies only to information collected through the Site and not to information collected offline or through any other websites (even if affiliated with Rehabwheel).

 

Please read this Policy carefully, because by using the Site, you are acknowledging that you understand and agree to the terms of this Policy, and consent to the manner in which we may collect, use and disclose the foregoing information. If you do not agree to the terms of this Policy, please do not use the Site.

What Information Do We Collect?

There are two kinds of information websites can obtain about you:

 

(1) Anonymous Information: “Anonymous Information” is aggregate data that websites use to administer their sites. It may include such information as your browser type or your Internet Protocol (IP) address (“Usage Information”), as explained in greater detail herein. Rehabwheel may from time to time retain third parties to help us collect and aggregate Site customer information.

 

Device Identifiers. In the course of collecting Usage Information we may also collect your IP Address, MAC Address or other unique identifier (each a “Device Identifier”) for the computer, mobile device, Wi-Fi card, or other technology (collectively, “Device”) you use to access the Site. A Device Identifier is a unique string of numbers that is automatically assigned to your Device by your Internet Service Provider. Web servers automatically identify your computer by its Device Identifiers. When you visit the Sites, we may view your Device Identifier. We use this information to identify repeat visitors to our Site. We also may use this information to send you targeted advertisements and to enhance the Site, such as helping to diagnose problems with our server, to administer our Site, avoid hacking attacks on a Site’s server. We may also use your Device Identifier to gather broad demographic, geographic, and industry information. We do not link Device Identifiers to any personally identifiable information, even if such information is voluntarily provided by a user. We may associate your Device Identifier with your Personal Information.

Information Collected by Automated Means – Whenever you use the Site, we, as well as any of our third-party advertisers and/or service providers, may use a variety of technologies that automatically collect information about how the Site is accessed and used (“Usage Information”). Usage Information may include, in part, browser type, operating system, the page served, the time, how many users visited the Site, and the website you visited immediately before the Site. This statistical data provides us with information about the use of the Site, such as how many visitors visit a specific page on the Site, how long they stay on that page, and which hyperlinks, if any, they “click” on. Usage Information helps us to keep the Site fresh and interesting to our visitors and to tailor content to a visitor’s interests. Web Beacons – We may also include web beacons (also known as web bugs, Internet tags, pixel tags, tracking pixels and clear GIFs) with the content and ads that we deliver to you, which we will use to collect information regarding your interaction with our content and ads. A web beacon is a transparent graphic image placed on a web page or in an email, which indicates that a page or email has been viewed or that an email has been forwarded. In addition, a web beacon allows third parties to obtain information such as the IP address of the computer that downloaded the page on which the beacon appears, the URL of the page on which the beacon appears, the time the page containing the beacon was viewed, the type of browser used to view the page, and the information in cookies set by the third party. A web beacon may also tell your browser to get content from another server.

 

Cookies and web beacons (together, “Site Cookies”) may enable us to track and target the interests of our users to enhance the experience on our Site, track user actions/behavior on our Site and track the effectiveness of ads.

 

(2) Personally Identifiable Information: “Personally Identifiable Information” is any information that personally identifies you, such as your name, email address or your telephone number. Sometimes this personal information is needed when you submit a request for an appointment or when you contact us via email or online forms. We only have access to and collect personal information (such as your name, date of birth, social security number, credit card number, mailing or billing address, telephone number, or email address, and email preferences (collectively, “Personal Information”)) that you voluntarily give us via email or other direct contact from you. We will not sell or rent your Personal Information to anyone. We are the sole owners of the Personal Information collected on this Site. We may ask you to provide us with personal information on a voluntary basis in certain areas of our Site. In particular, you may be asked to provide the following information:

 

Email Correspondence. Scattered throughout the Site are links or forms that can be used to contact Rehabwheel so you can comment, make a complaint, make suggestions and ask questions. Your contact information, such as name, address, email address and telephone number may be required on these forms so we can address our answer in a mannerly way. Rehabwheel may put such information on file, but does not currently share, sell, license or transmit this information with third parties without express authorization from you.

 

Newsletters and Other Communications. We offer visitors to the Site the opportunity to sign up to receive newsletters or other email communications from the Sites. In order to sign up for these communications, we may ask for contact information, such as name, address and email address. We also may offer the opportunity for you to receive mailings from our partners that we think may be of interest to you.

 

Social Networks. The Site from time to time may offer the opportunity to publicly comment or share information through the use of third-party social media services and social networks (i.e. Twitter or Facebook). The Site may also enable you to post content from some of our sites to your profile on such networks. These networks should authenticate your identity and allow you to post and share certain personal information with their sites. Should you provide information in connection with these services, your username may also be required and made publicly available. Please be aware that the privacy policy of that site will apply to any content you provide in such instances.

 

How Do We Use Your Information?

 

We will use your information to respond to you regarding the reason you contacted us. We will also use your information as follows:

 
Registration – A user may need to first complete a registration form in order to use the Site. During registration, a user is required to give certain information (such as name and email address). This information is used to contact you about the products/services on our Site in which you have expressed interest. At your option, you may also provide demographic information (such as gender or age) about yourself, but it is not required.
 
Orders – We may request information from you on our order form. To buy from us, you must provide contact information (like name and shipping address) and financial information (like credit card number, expiration date). We offer you choices regarding the collection, use, and sharing of your Personal Information. If you receive promotional communications from us, you may indicate a preference to stop receiving further promotional communications from us and you will have the opportunity to “opt-out” by following the unsubscribe instructions provided in the promotional email you receive or by contacting us directly at the email address or phone number given on our Site. We do not charge for this service, and your opt-out request will be processed within 30 days of the date on which we receive it.

CONSENT TO PRIVACY POLICY
 
This Policy and the privacy practices of Rehabwheel will be subject exclusively to the laws of the State of California in the United States. Any claim related to the Sites or this Policy shall be brought in a federal or state court in San Francisco, California, within one year after the claim arises. Users of the Sites consent to the exclusive jurisdiction and venue of such courts as the most convenient and appropriate for the resolution of disputes concerning this Policy. This Policy and the notices outlined herein are not intended to and do not create any contractual or other legal rights in or on behalf of any third party.
 
Rehabwheel makes no representation that this Policy and our practices comply with the laws of any other country or jurisdiction. Users of the Site who reside outside the United States do so on their own initiative and are responsible for compliance with local laws, if and to the extent local laws are applicable. If you reside outside of the United States, by using the Site, you consent to the transfer, processing and use of your information outside your country.
 
SPECIAL NOTICE TO CALIFORNIA RESIDENTS
 
Under the California Civil Code, residents of the State of California that have provided any Personally Identifiable Information to Rehabwheel have the right to request a list of all third parties to which Rehabwheel has disclosed Personally Identifiable Information during the preceding year for direct marketing purposes. Alternatively, the law provides that if Rehabwheel  has a privacy policy that gives either an opt-out or opt-in choice for use of Personally Identifiable Information by third parties (such as advertisers or affiliated companies) for marketing purposes, Rehabwheel may instead provide you with information on how to exercise your disclosure choice options. Rehabwheel  qualifies for the alternative option; it has established this Policy that provides you with details on how you may either opt-out or opt-in to the use of your Personally Identifiable Information by third parties for direct marketing purposes.
 
OUR COMMITMENT TO CHILDREN’S PRIVACY
 
Protecting the privacy of children is very important to us. The Children’s Online Privacy and Protection Act of 1998 (“COPPA”) defines a “Child” as anyone under the age of 13. PMP strictly adheres to COPPA. For that reason, we do not collect or maintain information obtained through our Sites from those we actually know are under 13, and no part of our Sites are structured to attract anyone under 13.

MODIFICATIONS TO PRIVACY POLICY
 
Rehabwheel may modify this Policy at any time, at its discretion, and modifications are effective upon being posted on the Site. You are responsible for reviewing this Policy periodically to ensure that you are aware of any changes to it.
 
ACCOUNT REVISIONS, REVOKING AND DELETING PERSONAL INFORMATION
 
At any time, you may ask us to cease further use of your email address to send you correspondence and direct us to delete any Personally Identifiable Information you have supplied. Although it is not always possible to remove or modify such information, we will make reasonable efforts to do so. If you have given us information for one of those third parties we mentioned above and we have already passed it on, we may not be able to delete or change the information.
